ABSTRACT

Objective:To investigate the influence of echinoside on the expressions of inducible nitric oxide synthase (iNOS) and transforming growth factor-β1 (TGF-β1) in renal tissues of diabetic nephropathy rats.Methods:A total of 60 male SD rats were randomly divided into the normal group, model group, glibenclamide group and low-, medium-, high-dose echinoside treatment group. the rat model of diabetic nephropathy was established through the streptozocin induction and the glibenclamide treatment was applied as control. After molding, glibenclamide group was given 5 mg/(kg?d) of glibenclamide, and low-, medium-, high-dose echinoside treatment group was given 50, 100 and 200 mg/(kg?d) of echinacoside solution. The normal group and model group were intragastrically given equal volume of normal saline. After 4 weeks, the change rate of body weight, kidney weight/body weight, blood glucose, serum TC, TG, BUN, SCR and 24 h urinary protein levels were measured, and the NO release concentration in renal tissue cell culture medium was measured, the renal pathology was observed by HE staining, the expression of iNOS and TGF-β1 proteins in renal tissues was detected by Western blotting.Results:Compared with the model group, the body weight of rats in the medium-dose group significantly increased ( P<0.05); the weight change rate and kidney weight/body weight of rat in the medium-, high-dose group significantly decreased ( P<0.05), and the NO release concentration in the medium-, high-dose group decreased ( P<0.05); the level of blood glucose, TC, TG, BUN, SCr, and 24-hour urinary protein in the low-, medium- and high-dose groups significantly decreased ( P<0.05), and iNOS in renal tissue of rats in the low-, medium- and high-dose groups was significantly lower ( P<0.05). Compared with the model group, the relative expression levels of TGF-β1 (1.35 ± 0.06, 1.15 ± 0.05, 0.91 ± 0.04 vs. 4.58 ± 0.09) and TGF-β1 (1.24 ± 0.04, 1.41 ± 0.05, 0.89 ± 0.04 vs. 3.04 ± 0.05) in the low-, medium- and high-dose groups significantly decreased ( P<0.05). Conclusions:The echinoside can prevent weight loss, reduce blood glucose and triglyceride, inhibit the production of NO in renal tissues, reduce the expression of iNOS and TGF-β1 among diabetic nephropathy rats. It has been proved to play a protective role in diabetic nephropathy pathological tissues.

Objective To explore the medication closed-loop management of huangshi central hospital based on electronic medical records.Methods Diversified pharmaceutical information platform was constructed.The full coverage of all aspects of medication information,such as ordering/prescribing,transcribing,distributing,preparing,dispensing,administering,documenting,and monitoring was realized.Results The medication management and use had no information gap,and bias could be controlled to form a closed-loop management.Conclusion Application of the new information technology in the field of pharmaceutical care can improve work efficiency,reduce medication errors,broaden the vision of clinical pharmacists and scope of work,and improve the overall level of pharmaceutical care.

ABSTRACT

Objective To analyze the associations of the interleukin-10 (IL-10) promoter-1082G/A and-819C/T single nucleotide polymorphism (SNP) and serum level of IL-10 with intracranial aneurysm (IAs). Methods The polymerase chain reaction (PCR) and DNA direct sequencing methods were used to detect IL-10 gene promoter district two SNP site,-1082G/A and-819C/T genotype frequency and allele frequency in 206 patients with IAs and 187 controls. Chi-square test was used to analyze differences between two groups. The serum level of IL-10 was analyzed by ELISA, and t-test was used to analyze significant differences between two groups. Results There were significant differences in genotypes of GG and GA+AA, as well as the alleles G and A, in-1082G/A locus between IAs group and control group (P<0.01). There were higher frequencies of genotype GA+AA and the allele A in IAs group than those in control group (P<0.01). There was higher risk of suffering IAs in patients with genotype GA+AA (OR=4.137, 95%CI:2.476-6.914) and the allele A (OR=3.368, 95%CI:2.476-4.583). There were higher frequencies of the genotype CT+TT and the allele T in-819C/T locus in IAs group than those of control group (P<0.01). There was higher risk of suffering IAs in patients with genotype CT+TT (OR=3.393, 95%CI:1.952-5.900) and the allele T (OR=3.764, 95%CI:2.730-5.192). The serum level of IL-10 was significantly lower in IAs group than that of control group (P<0.01). Conclusion The IL-10 promoter SNP influences the expression of IL-10. IL-10 promoter-1082G/A and-819C/T polymorphisms are correlated with the formation of IAs.

ABSTRACT

Objective To investigate the curative effect of Candesartan on chronic systolic heart failure in Tibetan pla?teau. Methods A total of 526 patients with chronic systolic heart failure were divided into two groups randomly (Treatment group N=252;Control group N=274). Regular medicines, such as cardiac, diuretic and vasodilator drugs , are given to pa?tients in both groups according to their primary underlined diseases. Additionally, enalapril is added in control group, while candesartan is added in treatment group. Improvement of cardiac function, LVEF, LVEDD and blood pressure were compared between these two groups after 8 weeks. Results There was no significant difference in effective rate (i.e., 45.2%in treat?ment group and 44.9% in control group, respectively), efficiency rate (i.e. 54.8% in treatment group and 55.1% in control group, respectively) and inefficiency rate (i.e. 2.0%in treatment group and 2.2%in control group, respectively) between treat?ment and control groups . After treatment, LVEF in the two groups are all improved, while LVEDD and blood pressure both de?creased with statistical significance. However, the difference between the two groups is not significant. Conclusion No sta?tistical difference was found in curative effect on chronic systolic heart failure in Tibetan Plateau between two groups.

ABSTRACT

Objective To investigate the significance of galactose galectin-3(Galectin-3)and matrix metalloproteinase-2(MMP-2)expression in gliomas patients and thire role in process of gliomas' malignancy development. Methods Immunohistochemistry was used to detect Galectin-3 and MMP-2 protein expression in 5 normal brain tissue and 40 patients with different grade gliomas. According to positive cells number of Galectin-3,MMP-2 in tumor cells under a microscope,to determine the expression,and the positive index(LI)which came from the percentage of the positive cell number out of the total cell number was to expressed the number of positive cell. Results Galectin-3 and MMP-2 protein expression in normal brain tissue were negative. In glioma tissues,Galectin-3 was mainly expressed in the cytoplasm and membrane of tumor cells. In 23 glioma tissue withⅠor Ⅱ grade,9 cases(39. 13% )was positive and the LI values was(5. 65 ± 3. 47)% in terms of Galectin-3 expression. In Ⅲ,Ⅳ grade glioma specimens,the positive rate of Galectin-3 expression was 76. 47%(13 / 17), and LI value was(27. 88 ± 22. 13)% . The difference of Galectin-3 expression and LI value were significant between specimens with Ⅰ,Ⅱ grade and Ⅲ,Ⅳ significant( χ2 = 4. 101,t = 4. 105;P < 0. 05). In human gliomas,MMP-2 expression protein was mainly expressed in tumor cells and vascular basement membrane of the endothelial cell cytoplasm. In 23 glioma tissue with Ⅰor Ⅱ grade,9 cases(39. 13% )was positive and the LI values was(5. 91 ± 4. 78)% in terms of MMP-2 expression. In Ⅲ,Ⅳ grade glioma specimens,the positive rate of MMP-2 expression was 88. 24%(15 / 17),and LI value was(30. 06 ± 22. 94)% . The difference of MMP-2 expression was significant between specimens with Ⅰ,Ⅱ grade or Ⅲ,Ⅳ grade( χ2 = 7. 882,t = 4. 271;P< 0. 05). The linear correlation analysis showed that there was positively correlation between Galectin-3 and MMP-2 positive cells(r = 0. 800,P < 0. 05). Conclusion Galectin-3 and MMP-2 protein expression in Ⅰ,Ⅱgrade gliomas is significantly lower than those inⅢ,Ⅳ grade glioma,and they are positively related with the progress of malignant gliomas. Galectin-3 and MMP-2 protein can be used to evaluate or judge the malignant stage of human brain glioma.

ABSTRACT

BACKGROUND: Levodopa substitutive therapy is essential and effective method for PD for a long time, but it can not delay the course of PD. Side effect is more and more and severe, for taking levodopa long term. It is benefit to patients to postpone the usage of levodopa or decrease the dosage of it.OBJECTIVE: To systematically investigate the curative effect of the selfmade Bushenpingchan formula combined with madopar on pathogenetic condition of PD patients after onset.DESIGN: Randomized controlled observation.SETTING: Senile Disease Research Center, Xuanwu Hospital, Capital University of Medical Sciences.PARTICIPANTS: Seventy patients diagnosed as having PD were recruited from the Xiyuan Hospital of Chinese Academy of Traditional Chinese Medicine between December 1999 and May 2002. Inclusive criteria: The patients must be diagnosed as PD finally; ②be at 1.5-4 grade of modified Hoehn & Yahr measuring scale; ③less than 80 years old; ④The Traditional Chinese Medicine (TCM) diagnosis was deficiency of liver and kidney. Exclusive criteria: ①secondary PD; ②accompanying with other sever central nervous system diseases. ③patients with severe diseases of heart,lung, kidney or multi-viscera-failure; ④mental sickness patients; ⑤drug and alcohol abuse; ⑥severe adverse effect.METHODS: Totally 70 patients were divided into two groups based on the proportion of one to one: There were 35 patients in treatment group containing 22 males and 13 females. The average age was (67.9±16.5)years. There were 35 patients in control group including 25 males and 10females. The average age was (65.5±16.5) years. There was no significant difference in their general condition. Patients in the control group received 62.5-500 mg madopar (250 mg in each pill), 2-4 times a day (to change the dose according to patients' condition). The patients in the treatment group were given with 62.5-500 mg madopar (250 mg in each pill), 2-4times a day orally and 200 mL self-made Bushenpingchan formula (twice a day) in addition, which was composed of 20 g tuber fleeceflower root, 1 g hairy deerhorn, 10 g tall gastrodia tuber, 15 g gambirplant, 12 g Chinese thorowax root, 15 g twolobed officinal magnolia bark, twice a day. The course of medication was 3 month in patients of the two groups. The patients were determined with unified Parkinson disease rating scale (UPDRS) before treatment and at weeks 2, 4, 8 and 12 after treatment, respectively. The symptoms in traditional Chinese medicine after treatment were measured. The dose of madopar before and after medication as well as the adverse effect were observed.MAIN OUTCOME MEASURES: Score of UPDRS before treatment and at weeks 2, 4, 8 and 12 after treatment in patients of the treatment group and control group; the dose of madopar before and after treatment and the adverse effect after treatment.RESULTS: Total effective rate was 69% and 51% in the treatment group and control group, respectively after treatment. There was no significant difference in therapeutic efficacy (x2=0.86, P=0.222 3). ①The score of UPDRS reduced significantly 8 weeks after treatment in the treatment group as compared with that before treatment (t=2.18 ,P < 0.05). ②The daily dosage decreased in the patients of the treatment group after treatment as compared with that before treatment, which had significant difference (t =2.862 8,P < 0.01 ). The daily dosage in the patients of control group had insignificant difference with that before treatment (t=2.320 3,P < 0.01 ). ③There were 2 cases with lightly dry mouth, nausea, dizziness,which could relieve two weeks later in the treatment group; 5 patients with nausea and 2 patients with light dizziness in control group, which could relieve. The blood pressure had no significant change in the two groups before and after treatment.CONCLUSION: The Chinese medicine Bushenpingchan formula combined with western medicine can decrease the UPDRS score in PD patients,can improve the syndromes in Chinese medicine.
